Booking Online is Easy
- Click the Link
- Choose your treatment or service & any add-ons to customize your service.
- Choose your provider.
- Choose your date & time.
- Click “Book Reservation”
- Sign into your account (or create a new account)
- Optional to add a note for your preferences & to include important information.
- “Make a payment” is not actually making a payment, it is just requiring a credit/debit card to hold the appointment. You will be charged for your services once they have been completed.
- Confirm and complete intake/consent forms
Collab's Cancellation Policy
We get it—life happens! But because your appointment time is reserved just for you, we ask for at least 24 hours’ notice if you need to cancel or reschedule.
- To cancel: Please call or text us directly at (480) 400-0061 or reply to your confirmation text message.
- To reschedule: You’re welcome to do this yourself through our online booking portal or call and we can help you.
- Late cancellations & no-shows: Cancellations made less than 24 hours before your appointment, or missed appointments, will be charged a $25 fee per appointment to the card on file.
A valid card is required to book, but it will only be charged in the event of a late cancellation or no-show. We appreciate your understanding—it helps us keep appointments available for everyone!
