We want to make sure we’re ready for everyone joining us, so please Register before the event.
If you’re bringing a spouse, friend, family member, or other guest, please register them separately using the RSVP form. Each guest needs to be registered to participate in the event activities, food, drinks, and raffle giveaways.

Official Terms & Conditions
Collab Medspa is celebrating 10 years in Scottsdale! During our 10-Year Anniversary Celebration on Thursday, October 22, 2026, from 5:00–7:00 PM, guests will have opportunities to receive entries for our anniversary prize drawings.
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
Eligibility
Prize drawings are open to guests 18 years of age or older. Employees of Collab Medspa and members of their immediate households are not eligible to win. Void where prohibited by law.
How to Receive Entries
Guests will receive an initial entry by attending and registering for the Collab Medspa 10-Year Anniversary Celebration.
Additional entries may be available at no cost by visiting participating vendor, sponsor, or affiliate tables and completing designated free activities during the event.
No purchase, payment, appointment booking, or deposit is required to receive entries.
All entries must be submitted before the drawings begin.
Choosing Your Prize Drawings
The celebration will feature 10 or more individual prizes, with a separate drawing for each prize.
Guests may choose how to distribute their entries. You may place all your entries into one prize drawing or divide them among several different prizes.
Each entry may be used for only one prize drawing. Once submitted, an entry cannot be moved to another drawing.
The odds of winning each prize depend on the number of eligible entries submitted for that particular prize.
Prizes
Prizes may be provided by Collab Medspa and participating vendors, affiliates, and sponsors and may include treatments, skincare products, gift certificates, merchandise, services, and other anniversary gifts.
Individual prizes may be subject to restrictions, expiration dates, eligibility requirements, or scheduling requirements established by Collab Medspa or the business providing the prize.
Collab Medspa treatment and service prizes remain subject to our normal consultation, medical eligibility, safety, contraindication, and scheduling requirements. If a winner is not an appropriate candidate for a treatment, Collab Medspa may substitute another Collab Medspa service of comparable value when appropriate.
Third-party prizes are fulfilled by the business providing the prize and may be subject to that business’s terms and restrictions.
Winner Selection
The prize drawings will take place during the anniversary celebration at approximately 6:30 PM on Thursday, October 22, 2026.
One winner will be randomly selected for each prize from the eligible entries submitted for that prize.
You do not need to be present to win.
Participants may enter multiple prize drawings and may win more than one prize.
Winners who are not present will be contacted using the information provided during event registration. Winners must claim their prize within seven (7) days of notification. If a winner cannot be contacted, does not respond, is ineligible, or declines the prize, Collab Medspa may randomly select an alternate winner.
Event Specials
Collab Medspa may offer event-only specials, discounts, appointment-booking opportunities, or other promotions during the celebration. These offers are separate from the prize drawings and do not provide additional entries or increase a guest’s chances of winning.
Social Media & Privacy
These prize drawings are sponsored and administered by Collab Medspa and are not sponsored, endorsed, administered by, or associated with Facebook, Instagram, Meta, or other social media platforms.
Information collected for the drawings may be used to verify entries, contact winners, and fulfill prizes. Participation does not require consent to future marketing communications.
Collab Medspa will never require a winner to pay a fee or provide credit card or payment information to claim a prize.
General Conditions
By participating, guests agree to these Official Terms & Conditions.
Collab Medspa reserves the right to disqualify fraudulent, duplicated, altered, or otherwise invalid entries and to modify, postpone, or cancel a drawing if circumstances beyond its reasonable control affect the fairness or operation of the drawing.
If a prize becomes unavailable, Collab Medspa may substitute a prize of comparable value when appropriate.
